  1. Review the eligibility tools and assessment measures in the Local Coverage Determinations (LCDs) and how they can help – or hinder – your eligibility documentation

  2. Learn how to document Activities of Daily Living in a manner that is consistent from clinician to clinician
  3. Review areas of documentation that are necessary to prove eligibility
  4. Develop a tool to track all essential areas for recertification for easy review and develop a plan for review of potential live discharges

Carol Javens serves as an Account Manager for Acclivity Health Solutions, where she focuses on strategic consulting to both providers and payers to build hospice and palliative care programs as well as use of data to drive compliance and patient identification earlier in the disease trajectory. She has extensive experience as a hospice nurse from fieldwork to director of nursing, including earning the prestigious certification in hospice and palliative nursing. Carol has spent recent years working in consulting, education, technology development and utilization for providers of all sizes, from startups to some of the largest hospice and palliative providers in the country
