There is strong evidence proving community-based palliative care can significantly impact quality of life, resulting in more compassionate, affordable, and sustainable high-quality care, including improving hospice use. Many hospital systems have palliative care programs, but after discharge the continuation of palliative care in a community-based program continues to be a challenge.
This webinar will address the benefits and value of a community-based palliative care program, including how palliative care can serve people during a national health crisis like COVID-19. It will examine key program principles and requirements and how to identify potential integration and growth areas. Payer source opportunities for program integration will also be covered. Characteristics and best practices of CBPC programs will be reviewed to help your agency benchmark for quality, policies, and training.
