The Center to Advance Palliative Care (CAPC) is a national organization dedicated to increasing the availability of quality health care for people living with a serious illness. As the nation’s leading resource in its field, CAPC provides health care professionals and organizations with the training, tools, and technical assistance necessary to effectively meet this need. 

BENEFITS OF MEMBERSHIP

CAPC is funded through organizational membership and the generous support of foundations and private philanthropy. It is part of the Icahn School of Medicine at Mount Sinai in New York City.

The rates for our 12-month CAPC memberships for hospices range from $700-$3,750, dependent upon ADC.  CAPC is offering an introductory 15% discounted membership rate off your first year of membership to California Hospice and Palliative Care Association members. The essential reference for planning and starting a home-based palliative care program, Palliative Care in the Home: A Guide to Program Design outlines a process for designing a program that provides the highest-quality care, is responsive to stakeholder priorities, and is financially and operationally sustainable.

Developed by the CAPC Home-Based Palliative Care Workgroup, an interdisciplinary committee of advisors that represents medicine, nursing, social work, and administration, Palliative Care in the Home shines a light on replicable principles and practices of some of the most successful home-based palliative care programs in the United States. Using case studies from three home-based programs representing different payment models and organizational platforms—a hospice organization, a health care system, and a large multispecialty clinician group—it offers key insights into the mechanics of these established programs. Palliative Care in the Home also provides tips from the field that reflect real-world challenges and opportunities. Symptom Management FREE Training

Improve patient quality of life and prevent avoidable crises for patients with serious illness through expert management of common symptoms.

Uncontrolled symptoms lead to preventable suffering and crises that often result in ED visits and hospitalizations for patients and their families. CAPC’s symptom management unit covers techniques for assessing and managing common sources of distress, including nausea, constipation, dyspnea, depression, and anxiety. Pain is a much-feared side effect of many serious illnesses, but is manageable with proper training.
