Value based purchasing success is dependent upon our field clinicians and case managers. This course will focus on how HHVBP brings opportunities to the agency to focus on the “things that matter”—keeping patients out of the hospital, improving function and patient and family satisfaction. This focus will afford agencies up to a 5% bonus (or penalty), and will certainly have a long-term financial impact.

  • Learners will be able to identify the components of HHVBP
  • Learners will be able to discuss strategies to reduce hospitalizations
  • Learners will be able to verbalize CMS guidance for the primary OASIS items impacting HHVBP
