Program Description:

Clinical Documentation is “telling the patient’s story” and clearly showing the value of the home care services. Few people see the patient and everyone else must read the story. To provide a foundation for CDI Programs, agencies must educate clinicians and provide continuous analytic feedback to staff and management. Documentation does not need to be excessive, but it must be accurate and defensible.
